FTDI is funded through employee payroll
deductions to State Disability Insurance. The estimated
average cost is $27 per worker per year, divided between
your monthly, biweekly or weekly pay period.
If you do not see
an SDI deduction on your pay stub, ask your employer
or human resources representative if you are covered
by the SDI program.
You may request a claim form online or by phone. Claim
forms cannot be downloaded or faxed.
